Peri-implant bone regeneration: an alternative way by collagenated porcine-fibrin sealant graft

  • cover image
  • Initial status
  • Intraoral X-ray: Post-extraction defect after 2 months
  • Crestal dehiscence
  • From surgical video: Upper inteference for small mouth opening
  • From surgical video: Lower interference of crown in site 4.4
  • Deeper placement of mesial implant to allow primary stability
  • Mixture of collagenated porcine/fibrin sealant (OsteoBiol® Gen-Os® and Tisseel, Baxter). An alternative way to induce the bone regeneration. The fibrinogen has inductive action. After polimerization the fibrin sealant has barrier effect
  • Horizontal mattrees sutures
  • Intraoral X-ray after surgery: The mesial implant is tilted due to interferences
  • Intraoral X-ray after 4 months: New density around the implant
  • Before and after
  • Histology 100x magnification: Particle surrounded from new mineralized bone (red) and osteoid tissue (light blue). Absence of necrosis and inflammation cells. (Prof. Giovanna Iezzi)
  • Histology 200x magnification: Osteoid tissue in mineralization process, with active osteoblasts, adherent to particle (Prof. Giovanna Iezzi)
  • Two months after second surgery
  • Titanium personalized abutments with “Vertical Finish Line Geometry”
  • Final restoration fixed by zinc oxide eugenol temporary cement. Any residues in the sulcus are dissolved after 3-4 weeks for action of salivary enzymes
  • Intraoral X-ray: The “Prosthetic Abutment Height” between implant platform and crown is 3 mm to reduce marginal bone loss
  • Clinical indication: dehiscences and fenestrations
  • Products : Gen-Os
